I declare, that I have permission to enter the personal data of the competitors, coaches and referees in the range - name, date of birth, weight, club membership and photography into an information system of the portal www.sutazeveslovanie.sk. These personal data will be used in competition organizations - in the creation of category occupations, in making drawings, in racing lists, in score sheets, in the creation and printing of diplomas, attendance sheets and registration cards. Lists such as race lists, category occupancy, drawings, scorers, diplomas, and attendance lists will include name and surname, age and weight category, and club membership. These lists will be publicly available. If there is the competitor who cancelled the consent or did not consent to the use of the above personal data, I will promptly inform the portal www.sutazeveslovanie.sk in writing to delete this competitor from the system. This affidavit with consent to the processing of personal data is valid for one year. After this period, I will be prompted to confirm this affidavit. I am aware of the legal consequences associated with this affidavit under § 15 of Act No. 122/2013 in Collection of Laws on Personal Data Protection Act as amended. Competitors wishing to be deleted from older scores or other reports where they are listed must send the written request. In the case of non-confirmation of this affidavit, it is not possible to use the www.sutazeveslovanie.sk portal because the personal data are directly necessary for the functionality of the system.
